West Kent Health and Care Partnership Residents' Charter questionnaire

The West Kent Health and Care Partnership is a collaborative initiative aimed at integrating health and social care services. We bring together local health and care providers, social services and community organisations to jointly meet the diverse needs of west Kent residents. We are dedicated to ensuring that our community thrives and that all residents receive the highest quality of services.

To achieve this goal, a residents' charter is being developed with west Kent residents and West Kent Health and Care Partnership organisations to foster good relationships between residents and local health and care services. It is aimed to clearly set out the level of input residents can expect when they are helping to shape our local health and care services.
