Social prescribing and community navigation is a service that can empower you to make a positive change in your life and within your community. It can link you to support for a range of different challenges that matter to you, including financial support, housing, information and advice, carers support and social connections and activities to help you live a happy, independent and safe life.
Link workers and community navigators have time to build trusting relationships, start with what matters to the person, create a shared plan and introduce people to community support.

We are now writing a strategy that will improve social prescribing and community navigation across Kent and Medway.

We needed your views and opinions about the service, which will help us write the strategy and continue to build a better service for our communities.
The survey closed on 27th March. Thank you for telling us your thoughts. We will study the responses we have received and write a summary of what we heard. This will be given to the team who are writing the social prescribing and community navigation strategy and they will use your feedback to influence the future of the service.
